A 68-year-old asymptomatic man with no significant medical or surgical history underwent a screening colonoscopy. Multiple 4- to 8-mm localized, pedunculated and semisessile, non-bleeding benign-appearing polyps (A) were incidentally found in the terminal ileum, and a biopsy specimen was taken. Histo-logic sections showed intramucosal lymphoid aggregates atop a stalk of smooth muscle and submucosal vessels (B). Higher magnification (C) showed germinal center formation (arrows) within the lymphoid aggregates, a feature reassuring for benign lymphoid hyperplasia. Due to the asymptomatic history and the lack of signs or symptoms of intussusception, prolapse, or bleeding, no specific colonoscopic follow-up is indicated.
