Endoscopic 'retroperitoneal fatpexy' of a large ERCP-related jejunal perforation by using a new over-the-scope clip device in Billroth II anatomy (with video)

ERCP has a known perforation rate of 0.3% to 1.3%. We describe a jejunal perforation in Billroth II anatomy during ERCP that was sealed with an over-the-scope clip device and managed conservatively.An 85-year-old woman with jaundice resulting from obstruction of the common bile duct caused by pancreatic cancer underwent ERCP. The patient's medical history included a Billroth II gastrojejunostomy 35 years earlier for duodenal ulcer. Cholangiography revealed a stricture in the lower-middle bile duct.
