Musculoskeletal: what's different in children? Pushed by a friend, foot-rolled, now can't walk.

This 7-year-old boy was playing with his friends. As usual, they were having a good time, but then all of a sudden one of his friends pushed him and in an effort to remain upright, his foot rolled over and he twisted his foot and ankle. At first, there was not that much pain and he did limp a little; however, the pain got worse and so did the swelling and so the next day he was brought to our Pediatric Urgent Care Facility. When he arrived, he was walking on his heel. He did not want to place full weight on his foot. Otherwise, everything was normal except that there was considerable swelling and bruising over the dorsum of the foot. On palpation, tenderness was elicited over the distal metatarsals and with all of these considered, radiographs of the right foot with comparison of the left foot were obtained (Fig. 1).
