Simultaneous posterior fracture-dislocation of the shoulder and bilateral hip fracture secondary to hypocalcemic seizure: case report and literature review

Simultaneous bilateral fracture-dislocations of the shoulder and the hip are extremely rare and usually occur following a seizure. These lesions may be consecutive to excessive muscle contractions during tonic-clonic seizures [1-3] or electrical shocks [4]. Certain medical conditions may predispose to this rare mechanism by reducing bone strength such as renal osteodystrophy (ROD) in which uremia and hypocalcemia can cause bone demineralization [5]. There have been few reports on bilateral fractures of the hip following convulsion, but simultaneous bilateral femoral neck fractures associated to a posterior fracture-dislocation of the shoulder secondary to hypocalcemic convulsion are extremely rare. We report the case of a young man who sustained this rare injury following hypocalcemic seizure.
