Expert's summary:In this nonrandomized study, the authors prospectively compared the functional outcomes of 763 patients undergoing robot-assisted radical prostatectomy (RARP) or retropubic radical prostatectomy (RRP). All the procedures were performed by a single surgeon between 2007 and 2010. The robotic approach offered faster recovery of continence after the initial 132 cases, and faster recovery of potency even when considering the initial cases. Furthermore, a younger age, a longer membranous urethral length on preoperative magnetic resonance imaging, and a higher serum testosterone level were independent predictors of better functional results. The positive surgical margin (PSM) rate was slightly higher after the robotic approach, but this difference was not statistically significant. The authors concluded that patients had better functional outcomes after RARP than after RRP.
