Re: Impact of Previous Radiotherapy for Prostate Cancer on Clinical Outcomes of Patients with Bladder Cancer

The authors evaluated bladder cancer patients with a previous prostate cancer diagnosis (n = 144) and compared the clinico-pathologic outcomes of irradiated (brachytherapy and/or external beam, n = 83) and nonirradiated (androgen deprivation therapy, radical prostatectomy, and/or surveillance, n = 61) patients. Patients belonging to the nonirradiated group had bladder tumors diagnosed earlier after their prostate cancer diagnosis (median: 24 mo, interquartile range [IQR]: 2-87 vs 59 mo, IQR: 25-88; p = 0.007), had a lower proportion of high-grade tumors (77% vs 92%, p = 0.016), and muscle invasive disease (43% vs 70%, p = 0.001) as compared with those in the irradiated group. Similarly, after a median follow-up of 53 mo (IQR: 24-75), there was a trend toward a higher 5-yr cancer-specific survival (83% vs 73%, p = 0.07) in nonirradiated patients. Also, among patients undergoing radical cystectomy, nonirradiated patients had a higher rate of organ-confined disease (44% vs 25%, p = 0.1) as compared with irradiated patients. Hence, the authors concluded that irradiation of prostate cancer was associated with a delayed bladder cancer diagnosis and these patients also had a higher chance of being diagnosed with high-grade tumors and at a more advanced clinical stage.
