Marked eosinophilia with abnormal basophilic granules in acute graft-versus-host disease post-allogeneic haemopoietic stem cell transplant for acute myeloid leukaemia without CBFB-MYH11 mutation

We report the case of a 41-yr-old woman who presented with a 1-wk history of fatigue and sore throat. Two years previously, she was diagnosed with Stage 2C ovarian clear cell adenocarcinoma. She achieved remission after a hysterectomy, adjuvant chemotherapy with carboplatin and paclit-axel and pelvic radiotherapy. Her full blood count on this presentation showed the following: haemoglobin 86 g/L (MCV 103fL), white cell count 2.9 x 109/L (neutrophils 0.1 x 109/L) with 5% circulating blasts and platelets 143 x 109/L. No basophils or eosinophils were seen.
