Major improvements have been made in the management of acute coronary syndromes (ACS) during the last decades, leading to a dramatic decrease in mortality [1]. Yet, approximately 20% of patients with ACS experience recurrent ACS in the next 3 years. Since platelets play a central role in the pathogenesis of ACS, we hypothesized that patients with recurrent ACS may have an intrinsic increase in platelet responsiveness compared to patients in whom ACS is inaugural. We designed a prospective case-control study to test this hypothesis. We evaluated platelet function including shear-induced platelet aggregation (SIPA) in patients with recurrent ACS compared to patients who experience a first ACS.
