Severe intra-abdominal bleeding plus large pneumoperitonium following procedure of prolapsed hemorrhoids (PPH)

Hemorrhoids are common, and a significant proportion of patients who have hemorrhoids experience symptoms such as bleeding, pain, and itching. Procedure for prolapsed hemorrhoids (PPH) is a safe and effective technique indicated for the treatment of prolapsed internal hemorrhoids, with high success and Low complications rate. Complications, when they occur, are minor and may include anal pain and rectal bleeding. Rare, potentially life-threatening complications like massive hemorrhage and pelvic sepsis has been reported. We report a rare postoperative complication after procedure of prolapsed hemorrhoids which has not been reported before. Patient presented with severe abdominal pain and hemorrhagic shock post-PPH. He was found to have large amount of free air and blood in his abdomen due to rectal bleeding and perforation. He was treated emergently through exploratory laparotomy, evacuation of hematoma, rectal repair, and defunctioning loop ileostomy. Patient was discharged in stable condition.
