You are the flight surgeon at a military base in the southeastern United States when you receive an urgent call from your boss. There has been a volcanic eruption on a small island in the Caribbean that has resulted in significant damage. A majority of the buildings have been destroyed or critically damaged and are deemed unstable. Electricity, telephone services, and water/sewer services have been disrupted, and existing public health infrastructure is also severely damaged. A state of emergency has been declared by the United Nations (UN), and its members are preparing for a united response. On Day 3 of the disaster, the nation that governs this small island requests U.S. military assistance in providing humanitarian assistance and medical evacuation. Within 48 h you are deployed with an Expeditionary Medical Support/ Contingency Aeromedical Staging Facility to the island. Upon arrival you find a growing refugee camp of approximately 3000 displaced residents that has been set up by surviving local officials. Immediate challenges include providing first aid, shelter, and water. After meeting with the executive staff, it is decided that the first priority is to provide water to the displaced population.
