In August 2011 we published a meta-analysis entitled "Above- or below-elbow casts for distal third forearm fractures in children? A meta-analysis of the literature" in your journal Archives of Orthopaedic and Trauma Surgery [1]. In this article we conclude that below-elbow cast (BEC) is not inferior to above-elbow cast (AEC) so that this is a valid treatment option for distal third forearm fractures in children. Since the publication of this article, two new trials [2, 3] have been published on this topic, therefore justifying an update of this meta-analysis. Unfortunately, the large trial of Rahman and colleagues did mainly report on radiological outcome measures. In our primary meta-analysis, we stated that there were some limitations to our study; for example, the small number of patients that could be included. By including these two trials we could add 174 (108 (3) + 66 (2)) to the 300 already included patients.
