A steel foundry has been ordered to pay a £150,000 fine and £24,000 costs after an employee was killed when an abrasive disc fitted to a high-frequency handheld grinder shattered. Stuart Stead, who was a fettler at HI Quality Steel Castings, died on 7 March 2012 during routine work on a casting at the company's site in Sheffield. When the disc fitted to the grinder he was using exploded, the fragments were catapulted across his work bay and a shard went through his visor and hit him in the mouth.
