Water firm drowning followed HSE warnings

South West Water has been fined ￡1.8 million for failures that led to the death of an employee during routine maintenance. The fine equates to 1.1% of the company's latest pre-tax profit of ￡159.6 million, which it made on ￡506.4 million revenue in the year to 31 March 2016.
