In this feature, we look at how competence and communication issues led to a contractor's worker suffering a serious leg injury when a valve failed during a pressure test on a boiler. The incident resulted in a ￡1.35 million fine for the operator of the biomass plant, Sembcorp Utilities (UK), ￡37,500 for the contractor, R&A Kay Inspection Services, and ￡120,000 for a second contractor, Central Industrial Services (Northern) (CIS).
