A printing business has been fined £100,000 with costs of £35,000 for failures that ended in an employee falling through a skylight and dying six months later from his injures. The incident occurred on 31 October 2014 at IKO-Design's premises in Leicester after a maintenance worker and another employee had gone onto the roof to install a new chimney flue, which was required to re-site a heater. After the maintenance worker went to fetch a tool, the second worker, Ranyodh Singh, who had only been with the company for a few weeks and had volunteered to help his colleague, fell through the skylight.
