Awareness during cardiac surgery--a single-centre prospective clinical audit of 1060 patients.

The desire to prevent awareness remains a major driver for the development and use of processed electroencephalogram monitors (pEEG). Rather than simply introducing routine pEEG monitoring, the cardiac anaesthetic group at Waikato Hospital decided to investigate the incidence of awareness using a single-centre prospective audit measuring the local practice against an expected awareness rate of 0.2%. All patients undergoing cardiac surgery, off-pump or bypass cases, were considered for the audit. With Anaesthesia and Intensive Care, Vol. 39, No. 5, September 2011 respect to patient ethnicity, 74% were New Zealand European, 19% were New Zealand Maori, 2% other racial groups and 5% had no ethnicity noted. The average age was 62 years (SD 18), approximately 50% of the patients had on-pump cardiac artery bypass grafting (CABG) operations, 25% single valve replacements, 10% single valve and CABG, 2% off-pump CABG, and the rest a mixture of more complex or unusual surgeries (for example multiple valve replacements and ascending aorta plus valve replacements).
