In evaluating postoperative decreased kidney transplant function, the differential diagnosis includes medical and surgical causes. Hy-dronephrosis can be ascertained easily using real-time sonography, as can many perinephric collections, such as hematoma, lymphocele, urinoma, or abscess. Other considerations include acute tubular necrosis, rejection, and surgical complications related to arterial anastomosis, such as renal artery stenosis and/or occlusion. One additional consideration, although uncommon, is renal vein thrombosis. The addition of Doppler evaluation with color and duplex information allows the investigation for renal vascular problems and should always be included in the sonographic evaluation of these patients.
