The CD34+ cell count of peripheral blood by flowcytometric measurements is a predictor of the peripheral blood stem cell (PBSC) yield, however the method is costly and complicated. In this study, we evaluated the clinical utility of hematopoietic progenitor cell (HPC) measurements by Sysmex XN hematology analyzer, the latest multi-item hematology analyzer, compared with old-type XE analyzer. We enrolled 33 patients with hematological diseases undergoing chemomobilization. The HPC counts of blood samples (n=84) were measured with XN and XE analyzers. The CD34+ cell counts of blood and apheresis (Aph) samples were also measured by flow cytometry. The HPC counts obtained with XN analyzer (XN—HPC) were significantly correlated with the counts by XE analyzer (XE-HPC). The XN-HPC counts were more strongly correlated with the CD34+ cell counts (r=0.953) than the XE-HPC counts in blood samples. The area under the receiver operating characteristic curve for predicting successful apheresis (more than1X106 CD34+ cells/kg/Aph) for XN-HPC count was 0.878. At a cut-off value of 19///L for XN-HPC count, the sensitivity and specificity were 79.5% and 85.7%, respectively. XN-HPC count may be a useful and economical marker for predicting sufficient PBSC yields than XE-HPC count. [Original]
