TeamSTEPPS: Addressing Imaging Procedure Appropriateness

The Institute of Medicine s 1999 report, To Err is Human, emphasized communication barriers as a root cause of medical errors in the United States health care system. It is human nature to err, but it also is human nature to create solutions to problems, find alternative methods of working, and overcome medical discipline-specific challenges. The attention-grabbing report led to the development of several health care improvement initiatives and evidence-based programs; however, communication errors, patient safety concerns, and adverse medical events remain a problem across allied health professions. It is estimated that the number of deaths from medical errors is as high as 400 000 per year, with harm occurring at 10 to 20 times that rate.
