Key issues in performing Hazard and Operability (HAZOP) studies

The hazard and operability study is the most commonly used process hazard analysis method in the world today. Management of the risks from many thousands of processes depends on its correct use. This article identifies several issues that can adversely impact the quality of HAZOP studies. Poor quality studies may result in omitted scenarios and poor management of process risks. Recommendations are made to address the issues.
