Giant intracavernous carotid aneurysm with intraorbital extension: an unusual cause of unilateral proptosis.

We report herein a very unusual case of a giant intra-cavernous aneurysm with intraorbital extension, presenting with unilateral progressive proptosis and vision loss. A 78-year-old woman was admitted to our hospital with progressive, painless proptosis of the left eye and decreased vision over a period of 1 year. Her physical examination revealed nonpulsatile proptosis of the left eye with conjunctival hyperemia and sixth nerve palsy. There was complete loss of light perception in her left eye. Fundu-scopic examination revealed optic atrophy on the left side. The right eye was normal. Orbital MRI revealed a heterogeneous intraorbital mass lesion extending into the left cavernous sinus. The mass was measured 5.5><4x3 cm in size and filled the left retrobulbar space.
