The concept of humanitarian medicine has been expanding and growing steadily and has become more popular in recent decades. But where does the concept come from, what is it, and what is the role of a speciality such as neurosurgery? This paper addresses these questions from the perspective of a French Neurosur-geon who has been involved extensively in this field since 1972.The views reflect personal reflections on experiences in many places in Europe, the Middle East, Africa and Asia. It also draws on discussions with many colleagues in organisations such as ONG, Medecins sans Frontieres, the Red Cross, OMS, La Chaine de l'espoir and Medecins des Monde. Finally, I have endeavoured to keep my own view in perspective by reading again the following treaties: Un souvenir de Solferino-Nouvelle Edition l'age d'homme (poche Suisse) Lausanne 1986, La Question Humanitaire - Philippe Ryfman, collection "Grands enjeux, editions Ellipses 1999, Charity Business - Bernard Kouchner, Editions "Le Pre aux Clercs" 1986,and the different reports of Medecins sans Frontieres.
