Acute gangrenous cholecystitis caused by torsion of the gallbladder is a relative rare entity. Patients present with signs and symptoms of acute abdomen that are indistinguishable from those resulting from other causes. This condition which is consequence of specific anomalies in the anatomy of the gallbladder can occur in children and adolescents, but is most frequent in the elderly people, mostly women. The peak incidence occurs in patients between 60 to 80 years of age. The recent increases of such cases are related maybe to the fact that people are now living longer. We present the case of a 73-year-old woman with gangrenous cholecystitis due to torsion of the gallbladder. We review the etiology, clinical presentation, diagnosis and treatment of this condition.
