North Korea's health system is in a dire state as a result of chronic under-funding, according to an Amnesty International report that has reignited debate about health standards in one of the world's poorest and most secretive countries. The report describes dilapidated and poorly equipped hospitals, where surgery is done without anaesthesia and doctors are paid in cigarettes and alcohol.
