In Reply Drs Dell'Anna and Taccone propose 3 potential explanations for the lack of survival and neurological benefit in our study of the use of a rapid infusion of 4degC normal saline in patients resuscitated from out-of-hospital cardiac arrest: (1) un-deruse of in-hospital therapeutic hypothermia, (2) adverse he-modynamic effects of fluid infusion, and (3) a modest temperature difference between the control and intervention groups.
