In Reply Ms Weiner raises the possibility that our survey item, "Please rate the degree of responsibility (if any) each of these entities should have in reducing the cost of health care," could have been misinterpreted as, "What degree of responsibility do each of the following have for increases in health care costs?" Cognitive pretesting during survey development did not reveal such ambiguity, yet Weiner's hypothesis remains plausible and speaks to some inherent limitations of survey self-reports.
