Boots pharmacists asked to review processes after serious CD incidents

STEVE BANKS, superintendent pharmacist at Boots, has written a strongly worded letter to all Boots pharmacists asking them to review their processes urgently after two "very serious dispensing incidents" involving Controlled Drugs. The incidents occurred in two different Boots stores and in patients with "significant underlying health issues". In one case, the patient received diamorphine instead of the prescribed morphine and, in the other incident, a patient was supplied with fentanyl patches intended for another individual, writes Mr Banks.
