A DESIRE to keep patients safe is at the heart of what pharmacists and their support staff do. Everyone who works in pharmacy is in some way working towards making sure that people use high quality and effective medicines safely and appropriately on an individual basis. And yet there is one area where the risk of errors is unacceptably high: the movement of patients between healthcare providers.
