WHY are the simplest things so hard to achieve? When patients are admitted to hospital for elective surgery, for example, they are asked to take all their current medicines with them. Of course, in their anxiety they may forget, which makes it difficult for the doctor and pharmacist admitting them to ensure they receive the right medicines. Keeping a written list of the drugs they are prescribed is halfway there, but the list may not match what patients actually take or should be taking.
