Ridiculous working hours

I have read the decriminalisation of dispensing errors Briefing (PJ, 10/17 December 2011, . p675) and thought, at last, a bit of common sense here. The reasons for errors are many and varied but, unless you happen to have a pathological hatred of patients, none is intentional. I think most of us over a lengthy career in pharmacy have had several close shaves over the years and tend to believe that "there, but for the grace of God, go I". A series of circumstances can derail any system, no matter how well intentioned and apparently foolproof it may appear.
