Updated guidance for healthcare professionals on saying sorry has been produced by the National Patient Safety Agency.The framework - called "Being open" - is supported by a Patient Safety Alert. It sets out 10 principles that should be followed when communicating with patients, families and carers when something goes wrong, such as acknowledgement, apology, multidiscipli-nary reponsibility and continuity of care.
