Benefits of lowering cholesterol in chronic kidney disease.

The SHARP study (June 25, p 2181)1 shows a significant reduction in major atherosclerotic events in patients with moderate to severe kidney diseasewith the combination of simvastatin and ezetimibe compared with placebo. Close examination of the life-table plot in figure 2 suggests a sudden rise in events at year 1, which corresponds to the rerandomisation of the simvastatin-only group to either the combination or placebo. Sudden statin withdrawal has been implicated in increased morbidity and mortality compared with continuation of statin in patients with acute coronary syndromes.
