Switching HIV therapies: competing host and viral factors.

Expert consensus on HIV-1 protease inhibitors-much like the broader question of when and in whom to start therapy-seems to swing back and forth. In The Lancet today, Joseph Eron and colleagues present unexpected results from two multicentre non-inferiority trials (SWITCHMRK 1 and 2), which were identically designed to assess whether switching of the protease inhibitor component of a treatment regimen to an integrase inhibitor would reduce adverse effects (especially hyper-lipidaemia) while maintaining viral-load suppression.
