A 47-year-old homeless man presented to a local emergency department with intermittent pain and a pins-and-needles sensation in his legs. One month earlier, pares-thesias had developed in his toes, which spread gradually to his shins. He did not have lower back pain, muscle weakness, fever, chills, weight loss, bowel or bladder dysfunction, nausea, vomiting, or fatigue.
