Re: Safety of no bowel preparation before ileal urinary diversion

The authors present the results of an 18-month, randomized, prospective trial comparing standard 3-day bowel preparation to no bowel preparation and only overnight fasting in 40 consecutive patients scheduled for radical cystectomy and ileal loop urinary diversion. The results add to a growing body of literature supporting elimination of bowel preparation. The authors looked specifically at differences in bacterial overgrowth but did not have a large enough cohort to comment on significant differences in postoperative complications such as prolonged ileus or superficial wound infections.
