Report: Piece of Pipe Stopped BP Blowout Preventer from Working

A piece of drill pipe prevented the doomed Deepwater Horizon rig’s blowout preventer (BOP) from properly closing and stopping the blowout Apr. 20 that led to the rig’s loss, 11 deaths and the nation’s worst maritime oil spill. That was the conclusion of a report released by the federal government Wednesday into the failure of the BOP - a five-story, 300-ton structure that sat on the wellhead of BP’s Macondo well.
