I read with great interest the articles by Mathews and Binnington on the use of granulated sugar and honey to treat traumatic wounds ("Wound Management Using Sugar" and "Wound Management Using Honey," January 2002). I would think that the sucrose would act as a food source, but much to my surprise other principles supersede this logic. I have several questions. Does one work better than the other? Do they have specific applications based on the insult? The sugar article appears to focus more on trauma, whereas the examples in the honey article seem to be more thermogenic in nature. Both articles include a good discussion comparing these treatments with more traditional methods but do not compare and contrast the sucrose-based treatments (i.e., sugar vs. honey). Finally, the hydrogen peroxide liberation in the honey-based treatment raised a concern in that only dilute concentrations of hydrogen peroxide are therapeutic; and if it is too concentrated, I believe that hydrogen peroxide above 2 percent is toxic to healthy cells.
