Primary cutaneous nocardiosis due to Nocardia vinacea: first case in an immunocompetent patient.

A 8 5-year-old Korean man presented with a lesion on his left forearm. He had received empirical antibiotics for about 30 days at a private hospital, but the lesion had gradually enlarged. He reported having scratched his arm on some wire about 40 days previously. He denied any fever or chills. He was otherwise healthy, and his family and personal history were noncontributory.On physical examination, a markedly tender, diffuse purulent, erythematous plaque was seen, which had exudation and crusting, and surrounding induration (Fig. 1). There was no lymphadenopathy, and the respiratory and neurological examinations were unremarkable.
