I read with interest the article by Krishnan and Lochhead in which they described the first report of DM1-related maculopathy associated with spontaneous bilateral foveal schisis and lamellar holes.I note that in their discussion they dismiss the appearance of the surface of the macular images as one that can be misinterpreted on OCT as an epiretinal membrane, particularly in younger patients. Comparing the ocular coherence tomography (OCT) images of the left eye 4 months apart (4a and 4b) there is a clear difference in the appearance of the surface of the macula. The higher signal seen in the later image is not present in the earlier image; if this were simply due to a younger person's ILM being more reflective, this would be expected to diminish with time rather than increase.
