Acanthamoeba lenticulata keratitis in a hard contact lens wearer

A 28-year-old keratoconic hard contact lens wearer presented after 3 days of pain, redness and blurred vision in the left eye. There was no recent history of swimming or corneal abrasion. He described good lens hygiene habits and never slept wearing lenses. His visual acuity was 6/6 in the right eye and 6/36 in the left. Slit-lamp examination of the latter revealed a dendritiform lesion in the mid-periphery of the infratemporal cornea associated with a larger area of anterior stromal oedema. The lesion stained with fluorescein and was surrounded by punctate epithelial erosions. The anterior chamber was not inflamed, and fundoscopy was normal. A clinical diagnosis of herpetic keratitis was made, and scrapings were taken. The patient was given aciclovir ointment and homatropine drops.
