Although therapy for acute coronary syndromes and chronic coronary artery disease has improved markedly in past years (1,2), resulting in a population-wide increase in life expectancy due to better survival after myocardial infarction (3), acute ischemia remains among the most common causes of sudden arrhythmic death (4). Preventing sudden arrhythmic death in patients at sufficiently high risk to warrant the implantation of a defibril-lator is simple (4). Patients whose risk for sudden death is only slightly elevated (e.g., those with preserved left ventricular function with or without known coronary artery disease) require other, safer and less invasive interventions for the primary prevention of sudden death (5). This clinical dilemma is not new but remains largely unresolved (6).
