Re: Early experience of hyaluronic acid gel for breast enhancement.

We acknowledge the important contribution of Inglefietd in reporting the early experience of hyaluronic acid gel for breast enhancement. When introducing a new product, the outcomes of such a large cohort is important to inform the debate on its extended use. However, in our experience, the management of 'minor' adverse events such as pain, lumpiness and product migration pose difficult clinical dilemmas.Two patients presented with painful breast lumps post-Macrolane~(TM) injection for breast augmentation. Patient A presented one year post-injection and patient B presented at 8 months. Both patients presented with symptoms of pain, lumpiness and asymmetry. Neither patient had undergone pre-operative breast mammography, ultrasound orMRI. Due to the severity and non-resolving nature of their symptoms, both patients requested removal of Macrolane and augmentation with silicone implants.
