Lack of hazard training and improper safety techniques were some of the root causes for the fatal accident that led to the February death of Christopher B. Stratton, a 34-yearold field service mechanic, at the Jap Creek mine in Walker County, Alabama, an accident report from the US Mine Safety and Health Administration said. Stratton, a field service mechanic with Thompson Tractor for 13 years, went to Jap Creek on February 1 to diagnose and repair an operational problem with a diesel engine on a Caterpillar D10R bulldozer.
