Three- and four-part proximal humerus fractures: open reduction and internal fixation versus arthroplasty.

An active, 55-year-old, right-handed man fell onto his right side. He presented with isolated right shoulder pain, extensive ecchymosis and swelling from the shoulder to the distal arm, and normal neurovascular function. Radiographs revealed a Neer 3-part proximal humeral fracture, with a displaced anatomic neck component (Fig. 1).
