A woman, aged 40, was investigated because of a 1-week history of upper abdominal pain and jaundice. Liver function tests were abnormal with an elevated serum bilirubin (7.5 mg/dL, 127 (imoV L), alkaline phosphatase (339 U/L), alanine aminotransferase (982 U/L) and aspartate aminotransferase (667 U/L). An abdominal ultrasound study revealed a dilated bile duct and dilated intrahepatic ducts. A computed tomography (CT) scan showed dilatation of the bile duct with tapering of the duct in the head of the pancreas (Figure 1). Endoscopic ultrasound showed two hypoechoic lesions, 8 mm and 25 mm in diameter, obstructing the distal bile duct. A bile duct stricture was confirmed by endoscopic retrograde cholangiopancreatography and drainage was achieved using an endoscopic stent.
