Commentary: Outback catheter in femoropopliteal subintimal angioplasty: new horizons in treating chronic total occlusions.

We read with great interest the work of Bausback and colleagues1 from Leipzig on the Outback catheter for femoropopliteal occlusions. The experience reported by the authors is of the highest level and can certainly open "new horizons" in the treatment of chronic total occlusion (CTO) in the infrainguinal arteries.Subintimal angioplasty (SIA) is a minimally invasive percutaneous technique for the re-canalization of occluded iliac and infrainguinal arteries.2"4 First described by Bolia et al.5 in 1989, this technique has been used increasingly as an alternative to lower limb bypass procedures in critical limb ischemia (CLI).6 It is usually performed under local anesthesia and is based on the creation of a subintimal channel by endoluminal dissection and angioplasty.
