Our current armamentarium for the endovas-cular management of lower extremity vascular disease has a wide variety of options, including balloon angioplasty, balloon cryo-plasty, nitinol stents, covered stents, laser atherectomy, and variety of conventional atherectomy devices. Soon this toolbox will likely further expand to include technologies that incorporate the use of antiproliferative drugs, likely with variable delivery mechanisms. The endovascular community currently lacks data that provide adequate guidance in aiding the clinician's choice of the best therapy for patient care. Specifically, we lack adequate randomized data depicting superiority of one tool over another, in whatever disease subset (anatomical or patient) that should be evaluated.
