A 40-year-old woman was scheduled for laser in situ keratomileusis (LASIK) (-2.00 -3.00 x 9, right eye; -2.75 -2.25 x 169, left eye). Preoperative best spectacle-corrected visual acuity (BSCVA) was 20/20 in both eyes. Scotopic pupil size (Procyon) was 5.95 mm in the right eye and 5.82 mm in the left eye. Figure 1 shows the preoperative topography.During the microkeratome cut (180 mum, 8.5 mm, Hansatome, Bausch & Lomb), the patient was very agitated and an incomplete flap occurred in the right eye; the flap started at 6 o'clock and proceeded to mid cornea. Thus, no laser ablation was performed; the flap was refolded, and LASIK in the left eye was cancelled.
