We read with interest the article by Tamura et al., but we do not concur with theirconclusion that "our findings show that QOV and QOL improved after cataract surgery." First, the aim of the study was to assess whether treating visual disorders improves the quality of vision and quality of life (QOL). It is of paramount importance to note that QOL can never be assessed simply by only the revised Hasegawa dementia scale (HDS-R) adopted in the study. The HDS-R, like the standard Mini-Mental State examination (MMSE), covers only some aspects of cognitive function; eg, orientation, attention, general knowledge, and brief assessment of memory.2 It neglects the major higher cerebral functions (frontal and parietal lobes) such as the presence of agnosia, apraxia, and executive function that are important constituents of QOL. A conclusion based on such an incomplete examination and biased data is thin. Alternative tests including clinicians' global impression of change exist.
