Gas-forced infusion prevents endothelial cell loss in phacoemulsification

Surge is one of the important factors that cause endothelial cell loss during phacoemulsification.1 Currently, especially with the coaxial method, the intraoperative surge rate has decreased significantly thanks to enhanced phacoemulsification systems.2 However, it can be a problem with the biaxial method. Several techniques to maintain anterior chamber stability during biaxial phacoemulsification have been described. Gas-forced infusion (air pump) is one that is successfully used to prevent surge.3 In our study, we did not need this technique to maintain anterior chamber stability. Our preferred phacoemulsification parameters (vacuum, 250 mm Hg; aspiration flow rate, 30 mL/min; bottle height, 110 cm) and instruments (20-gauge sleeveless phaco tip and 19-gauge irrigating chopper) successfully prevented the intraoperative surge.
